The Port Gamble S’Klallam Tribe is administering a community survey to gather information about the needs and opinions of adult tribal members and other adults who live on the reservation about various issues.
This information will be used to create a picture of the community as a whole, to guide and plan for services. We do not keep track of your name, address, or personal information that identifies you or your family. All respondents will be confidential, and all responses will be pooled together with the responses from other respondents.
Your participation in the survey is voluntary. You don’t have to answer any question you don’t want to, and you can stop the survey at any point. We expect the survey should take about 15-20 minutes.
The survey includes a household section that should be completed by only one member of the household. Every adult living on the reservation or enrolled PGST member can and should complete the other sections.
Each enrolled PGST member aged 18+ who completes the survey will receive a check for $25, and all participants will be entered into a Grand Prize drawing for a pair of Mariners tickets.
The deadline closes mid-May 2023.

NOTE: This survey is for all enrolled adult Tribal members and other adults who are residents of Little Boston. If you are not part of either of those groups please do not start this survey!
