The Enrollment Eligibility Committee is asking ALL enrolled PGST members, aged 18 and older, to complete a survey to help assess opinions on proposed changes to enrollment eligibility for future potential members.
Access the survey by going to https://www.surveymonkey.com/r/PGSTEnrollment.
Enrollment number or name and DOB are required in order to verify identity and access the survey. Everyone eligible to complete the survey who does so will receive a $25 gift card. These will be distributed weekly.
The deadline for completing the survey is December 31, 2022.
Eligible enrolled PGST members are encouraged to complete the survey online as soon as possible. Anyone who hasn’t completed the survey by December 1 will be contacted by a staff member with the option to complete it by phone or in-person.
